This is the highest repeatable 3DMark99 Max score I could get with my AMD K6-III+ machine with 128MB RAM and a Nvidia RIVA TNT2 Ultra. Nowadays I use a modified 3DFX Voodoo 3 for glide, but I still keep the Nvidia card as a backup.
I actually have an even higher score of around 4600 3DMarks, but have not been able to duplicate it.

The card seems to work fine in games so I think the hardware is okay.

Your hardware should be fine. Methinks it's drivers. I also have a Voodoo 1 and had an issue with 3DMark99 too until finding better drivers. Also make sure that you are running the benchmark at 640 x 480 with double buffering. (Almost) All Voodoo 1's are only equipped with 2 MB of frame-buffer and 640 x 480 with 16-bit color is usually their limit. The other 2 MB of texture ram cannot be allocated to the frame-buffer. The image is probably still going to be fuzzy since the Voodoo 1 was made in 1996 with a lower quality VGA output than most display adapters from the following years.

New results, this time with a Radeon 9250 PCI I took from a computer to do a component-by-component test (and got greedy and decided to bench it).

Slight glitches in 3DM99 at 250/200, more at 275/225 and a crash in 3DMark 99's Shooter test, with heavy corruption in 3DMark 2000 Helicopter. No corruption in 2000 at 250/200, getting up to a moderate amount around 266/208, which seems to be the best this card can do stably (though I think I'll keep it at 180/150 since it's basically no different). Since 266/208 doesn't net any extra performance, I'm skipping it for the 800x600 tests. To be honest, I would probably leave this card in there if this weren't The DOS PC (at least, until I get a K5 setup going), and the 2D Nvidia support wasn't important. I'm actually quite liking it so far, but the whole fog and texture thing is a bit of a dealbreaker for me on this particular PC, where D3D sub-7 is a very real factor given the processor.
I'm reminded of something someone said about the Radeon VII: it's not fast, but it's torquey. It's getting similar framerates at 640x480 as it is at 800x600 within a few digits.
Eventually I'll get the FX 5200 back in and re-test with the more advanced methodology, but for now this was fun.
