Darkseed (1992) runs in 640x350 16 colours due to H. R. Giger's demand. Quite unusual for an adventure game at the time. Still looks great for today's standards.

weird this was bumped for the usual 640x480 modes. SVGA640 was becoming mainstream by 94. This thread had more extreme examples at the first page

Interestingly I remember the graphic mode description from the original wfw3.11 Standard Video drivers:

640x480x16colors = VGA (ok with 256kb Video RAM)
640x480x256colors = SVGA (from 512kb Video RAM)
800x600x16colors= SVGA (ok with 256kb Video RAM)

To me early is everything DOS before 1994, so EGA Hi Res and VGA-16 color are most likely suspects, after Windows 95 it’s sort of moot
