Hello everyone!
I really need some help identifying a particular graphics card just by its IO shield/ports. Its a dual slot one (an important factor here) with dvi, vga and s-video in this order. The combination of ports was common around the year 2004 or so, but for the life of me, I cannot find one which is also dual slot, like the one on the picture below.
Many thanks already in advance!

DVI, VGA, SVideo (TV Out)

Probably an ATI card of some flavor.

ATI with TV Out was a gimmick in the mid 90s.

Dual slot *might* be radeon though.

The ports are in the wrong order to be this guy-- this is just to show the existence of dual slot occupancy flavors of these cards actually existing.

The grille type for the cooler at least matches.

Again, gonna suggest some variant of late ATI / Early Radeon.

Yeah, all of the cards I've been able to find so far with these ports and being dual slot, had the s-video one in the middle, just like the one in your link. Found more Nvidias myself though, among them also 5800 ultra (that would be a find, eh?), but not a single card with this exact setup.

I think it's either a HD2400 AGP or an 8600GT. Probably an OEM card they didn't want anyone using the slot below Just a guess!

I would think if it were FX5800 you'd see the inside of the fan shroud through the vents.

My guess would be something of 2004 - 2007 era. Cards before 2004 were single slot and cards after 2007 didn't have svideo. So likely some kind of GeForce 6/7/8 or Radeon X1000/HD 2000 series.

Looking at the design of the second slot bracket, it very well could be Gainward brand - like this 8600 GT https://www.classicvga.com/product-page/gainw … 0-gt-512mb-ddr2

I think this is IT! I knew this would be the best place to inquire (even though I couldn't even remember my original username from years ago). I should have posted this immediately, instead of wasting an embarrassing amount of time trying to suss it out myself. The problem is, you don't really have a comprehensive list for cards this old; sure, Techpowerup's database is great for those around 2010 and newer, but for older stuff, it's quite spotty or sometimes even non-existent. For instance, you only have 3 8600gt cards in there and none of them are from Gainward. I did initially suspect a card of this generation (the Asus one has the same configuration of ports, but is single slot), but when I wasn't able to get a match, I broadened my search, which just got me nowhere... Many thanks for this!
