I am 99% positive I have the same dx25 motherboard - no idea what it is but you need at least a floppy controller installed so it can successfully boot. Bios is accesses through a dos program give me some time to search my thread here...
As for the model/make I have no idea...

I assume you want to install more memory, you need either SIPP memory to stick to those sockets or you can insert in them normal 30pin simm sockets and use simm memory or replace those sockets with simm sockets.
