PCI ID would help confirm what chip it is as well. Nice find, whichever chip it is 😀
Put together a system to do some testing and used this video card so I could verify what it is. The PCI ID is:
VEN_104E&DEV_0217&SUBSYS_00000000&REV_30
That comes back as what I expected - OTI64217
Now to see if I can find actual drivers or if I have to use the drivers that say they work for this from a previous chip.
Edit:
Installed the newest driver I have found so far. It is for the Spitfire cards but detects this card correctly as the OTI64-217. This is really a Win95 driver. The mouse cursor at 640x480 is all corrupted. Changed the resolution to 1024x768 and the mouse cursor disappeared. Found that enabling cursor trails made it show back up again. Weird.
Still looking for a newer driver.

The mouse cursor behaves correctly in Win98SE with this driver.
On another note, I found the link to the pages for the never released WARP5 drivers... Sadly it was a password protected page and was not archived.
